Applying for a Canadian passport for a child

If you are applying for a passport for your child, below is a list of items that parents or guardians must provide:

* Completed Child Application Form (PPTC 155) to be filled by parent or guardian.
* Proof of Canadian citizenship for your child (original documents only).
* Two identical passport photos with the photographer’s name and address as well as the date of photography must be on the back of one of the photos.
* Proof of parentage or parental guardianship.
* All documents that refer to custody, mobility or access to the child.
* Any valid Canadian passport or travel document issued in the child’s name.
